Integrating of community health workers into the process of identifying individuals in need of rehabilitation services within their homes and facilitating their access to appropriate health facilities will be very fruitful. Bridging gap between the community and health services is crucial especial in resource constrain areas in the country.
By linking these clients to appropriate facilities, they help ensure that individuals receive timely and necessary rehabilitation care, ultimately improving health outcomes and improve quality of life. Involving of community health workers into the process will improved identification of rehabilitation clients, increased referrals to health facilities, enhanced continuity of care, and better overall outcomes for individuals requiring rehabilitation services in rural areas in Tanzania.
