The data source that can be use to gather the information on the current scenario are community, patient, household, facility ,District and family data, This sources of data provides information depending on the problems identified within the population that may put the at risk of a disease
I can start with Reach user level data because t helps in the goal and outcome followed by Evaluation for the activities implemented and use of facility level database to keep the data gathered were collected from various levels.
Another alternative can use is The RE-AIM user level data as it focusses on the larger population upon implementation considering actual focusses at each level of Effectiveness,Adoption,Implementation and Maintenance as discussed in the scenario
